As we approach the end of the 2025/2026 year, there are a few forms we need to collect from every tutor and student pair.
1. For the program evaluation, please use THIS FORM. If you have questions about using the linked form, please give us a call and ask for Paula.
2. We will also be handing out a physical copy of the student evaluation form to everyone in the coming weeks, but it is also attached below.
3. Please turn in your student’s ELLIS grid.
4. Finally, we will need a filled out attendance form from every tutor. This is the blue sheet you received at the beginning of the semester.
